1. Schedule an enrollment appointment.

2. Verify your eligibility.
You will need to bring documents to your appointment that verify your:
- Age
- Residence
- Income eligibility
One of our staff will interview you and go over your documents to see if you're eligible for our senior care services. Learn more about proving your eligibility
Other items to bring:
- Social Security card
- All medical and hospitalization cards (i.e., Medicare, Medicaid, prescription card, or other insurance cards)
3. Receive full membership benefits.
Once approved, you are now eligible for any and all of our senior care services and products. You'll receive a schedule of applicable co-pay charges.
- Your membership is valid for one year
- Bring photo ID with you to all of your appointments and for any services provided
